Open Water Diver Course Forms

There is a Medical Form and a Student Folder required for participants to complete. Provide the completed Medical Form and Student Folder to your instructor at your first course session.

1) Medical Form

For your safety and to avoid delays in your course:

  • Download and print the Medical Form below.
     

  • Carefully read the form and answer each medical history question with a concise "Yes" or "No". We are unable to accept medical forms with blanks, check marks, lines drawn down the columns, or "Y", "N", "X" or "N/A" as answers.​
     

  • Complete the STUDENT section on Page 2 of the form​.
     

  • If you can answer "Yes" to one or more of the medical history questions, then written clearance to dive, without restrictions or conditions, from a physician is required prior to any in-water activities. Immediately do the following:​

    • Take or send all 6 pages of the form to your physician (MD or DO).

    • Ask your physician to complete the PHYSICIAN section on Page 2 of the form and return the form to you.

    • Student Divers who are professional physicians may not sign their own medical form.

    • Make a copy of the completed form and put it in a safe place. You may need it in the future.

    • Bring the completed original form to your first course session.

  • Give your completed Medical Form to your instructor at your first course session. Otherwise, your training will be delayed.​

 

  • For questions regarding your fitness to scuba dive, please speak with your physician. You and/or your physician can also speak with Diving Medicine Specialists at Divers Alert Network at no charge. Contact the Divers Alert Network (DAN) non-emergency medical information line at 919-684-2948 (Monday-Friday, 8am-4pm CT). Referrals to local diving and hyperbaric medicine specialists are available.

2) Student Folder

  • If you registered for the course over the phone, you will need to stop by Scuba Frisco to pick up your Student Folder.

  • If you registered for the course in person, you will be provided a Student Folder at that time.

  • Read and complete the following sections on your Student Folder, then bring it to your first course session:

    • First and Last Name on top edge

    • Student Profile (No photo is needed.)

    • Emergency Contact Information

    • Diving Interests

    • Non-Agency Disclosure and Acknowledgement Agreement

    • Standard Safe Diving Practices Statement of Understanding

    • Liability Release and Assumption of Risk Agreement

    • PADI Open Water Diver Course Record and Referral Form (On the back): Name, Date of Birth, Sex, Address, Phone and Email.
